Yeah i've had that happen sometimes.

Someone said that you should have the desktop colour depth the same as HL, not sure if thats right though?

I think it happened more when i was running HL in 32 bit colour mode, but i couldn't be sure if thats the sole reason. (barely happens now i put it back to 16 bit though)
