It is indeed Foobar2k with UIPanels and FofR addon/skin

Requires a bit of effort to setup intitally with all the album art etc but in the end I like it.

The other sweet feature is the lyrics panel, will go and search a DB and it gets the lyrics and displays them. Has gotten most songs I have thrown at it.
